These changes, however, may not compromise the infection-enhancing activity of the Abs, as this activity is likely essential for the virus to overcome the fitness cost associated with the mutations. This creates a worrisome situation where highly infectious immune escape variants can cause severe disease and evade the inhibitory effect of PNNAbs.
Furthermore, as the immune response shifts from S variant-nonspecific humoral to non-S-specific cellular immune effectors, the infection-mitigating effect strengthens, but the immune pressure on viral infectiousness increases across all circulating variants. This leads to the dominant propagation of new immune escape variants that incorporate non-S-specific infection-enhancing mutations. These variants can rapidly spread and cause severe disease, even in the presence of declining PNNAbs.
The emergence of these highly infectious variants, such as JN.1, marks the beginning of selective immune pressure on the conserved infection-enhancing site within S-NTD. This immune pressure can eventually nullify the virulence-inhibiting effect of PNNAbs, leading to a sudden surge in severe disease and mortality within highly vaccinated populations.
It is important to note that the impact of these variants on the immune systems of vaccinated individuals is not yet fully understood by scientific experts or public health authorities. The recent surge in hospitalizations and deaths in highly vaccinated countries, likely due to the spread of JN.1, highlights the need for further research and vigilance in monitoring the evolution of the virus.
In summary, the large-scale activation of CTLs and the shift from humoral to cellular immune responses in highly vaccinated populations can lead to the emergence of highly infectious immune escape variants.
